For hotels, transport (train, bus or airline), perhaps a driver for a day or so. Anyone with recent experience?

I've not used them personally as I made my own arrangements when traveling to Myanmar but Santa Maria Travels & Tours has a good reputation:

There are a lot of travel agents in Myanmar

I suggest emailing some of good local travel agents and compare , that's better for you

Burma Senses www.burmasenses.com, Green Myanmar Travel http://green-myanmar.com , One Stop Myanmar Travel http://www.onestop-myanmar.com/... and I am quiet sure there are ton of Myanmar travel agents when you search on google, but do not trust on the search results. People now can use SEO to hack the rank, the names displayed on the first page searched are not always the best.

Email some and compare their rate and itineraries is most recommended.
