I have a Chase (United) card that expired in 04/13 but by the start of May, I still hadn't received a new card with an updated expiry date. Once I contacted them, they sent it right away:

I apologize for the inconvenience caused about the non
    receipt of renewal cards.

The new card will sent via "Rush Delivery" to the
mailing address for your account. You should
receive it in 2 business days.

Then I realized that I had yet another Chase (United) card that was due to expire this month (May 2013) and hadn't received it either, so I wrote once again. The answer surprised me:

I have reviewed our records and found that your cards were
ordered on 12/08/2011, but have not yet shipped as of
05/10/2013. We apologize for this inconvenience.

We have resolved this issue and you will receive your
cards within three to five business days.

EIGHTEEN months?? Maybe it was just a typo and she meant 12/08/2012? But still TWO cards in successive months not being automatically renewed and/or despatched.

Has anyone else had similar experience?

    Interesting:
Both my wife and I had Chase CO cards that expired 1/2015.
After the change-over/merger we got new UAL cards with the same numbers and expiration dates then about 6 months ago we got another set of cards with new expiration dates of 1/2016 :)

Same here... I had the legacy CO card, got the rebranded card with the same account details, then a few weeks before the actual expiration got the new card with new details. I've never experienced any issue like the OP, and both my girlfriend and I have the Chase UA cards.

Sorry, I gotta fess up before I get sued for slander! I discovered that I did have the Chase cards that I said that I hadn't received. They had sent them w-a-y in advance of when I needed them, so I had filed them away ... and forgot about them!! Now, if Chase had just said that they had already sent them rather than apologize for not having sent them, I might have found them....

I currently live in Japan, so only use by U.S. based cards for online purchases, so I don't even use the physical cards except when travelling.
